Diving incident reports

 

Diving incident reports should be on the reading list of any diver, professional or otherwise. They highlight the types of situations that commonly lead to accidents and help to identify chains of events that make situations worse, such as buddy separation. However, it’s not easy to draw any lessons from incidents because understanding the decision-making processes that led up to the event is not always clear.

 

It’s very easy to say “They shouldn’t have done this”, or “I would never have done that”. None of that helps to identify systemic failures and prevent future incidents from occurring.

 

The DAN and BSAC reports are as detailed as they can be with the information given. It clearly takes a lot of work to produce these reports so hats off to both organisations for continuing to do them.
